I attest that the following requirements are met: The e-bike is rated Class 1, Class 2, or Class 3. The e-bike battery carries a warranty of at least 1 year. Bike was purchased on or after April 15, 2022. The rebate will not exceed 50% of the purchase price. E-bike meets all requirements currently listed on the program website. E-bike is new. E-bike is not a full suspension mountain bike. E-bike is not built from a conversion kit. E-bike invoice is paid in full and applicant has possession of the bike. The incentive application is submitted within 180 days of purchase. Rebate recipient must retain ownership of the e-bike for a minimum of 12 months from the date of purchase. The applicant resides at the Ashland Electric service address listed on this application. The applicant has consent to apply from primary utility customer, or an authorized individual under the account. The applicant is the purchaser of the bicycle. Only one e-bike rebate may be issued per Ashland Electric Utility account. Submitted program application and accompanying documents become the property of the City. This vehicle was purchased at a state-licensed dealership or Original Equipment Manufacturer (OEM). If new, this vehicle is listed at time of purchase on OCVRP's Standard rebate eligible vehicles list. If used, this vehicle is listed at time of purchase on the OCVRP's Charge Ahead rebate eligible vehicles list. The vehicle is registered at the active Ashland Electric service address listed on this application. The vehicle will be retained by the applicant for a minimum of 24 months. The incentive is being applied for with the knowledge and consent of the primary Ashland Electric customer listed on this application. Only one electric vehicle incentive may be issued per utility account. Submitted program application and accompanying documents becomes the property of the City.
